Senior Care
The Senior Care Program helps frail,
health-impaired older persons select and locate the services that
can help them continue to live in their homes. Personal care,
light housekeeping, meal preparation, nursing care, shopping and
transportation are among the services available. Services are
made possible through the collaborative efforts of MAC
or the local Commission on Aging, the Department of
Social Services, and the local Health Department.
Each agency is
committed to improving the coordination of services and filling
gaps in home care for the elderly. After an evaluation is done in
the home, a case manager ensures that the older person receives
the right services at the right time.
To be eligible a person must:
 | be 65 years of age or older |
 | need assistance with several daily activities
(personal care, meal preparation, etc.) |
 | be
medically at risk of institutionalization |
Senior Care has some funds available to help
pay for gap-filling services for persons who meet established
medical and financial criteria.
MAC operates the Senior Care
Program in Wicomico County. The lead agency in the other three
counties are:
Dorchester County : The Dorchester Co. Dept. of Social
Services
Somerset County: The Somerset Co. Commission on Aging
Worcester County: The Worcester Co. Commission on Aging
