designed to help older persons maintain their independence and dignity
while they remain in their own home environment. The programs
include Senior Care, Home Aide Program, Meals-on-Wheels, and Housing

_Meals-on-Wheels
Older persons need proper nutrition to maintain good health. The Home-Delivered Meal Program, better known as "Meals on Wheels," provides a life-giving service to the homebound elderly who are confined to bed, lack transportation, or are too frail to cook for themselves. While delivering meals, the driver serves as a link to other community services and resources. The regular visits also provide reassurance and alleviate some of the anxieties of those elderly persons who live alone. Meals on Wheels is supported by the United Way.

_Home Aide Program
Our Home-Aide Program strives to maintain or improve the health of the elderly who are chronically ill or severely disabled and are at risk of institutionalization. In many instances, staff members are able to ease an older person's return to his home after hospitalization. Services performed by aides are under the direction of a registered nurse and include personal care, transportation, housekeeping, and arranging for home-delivered meals.
Medical Assistance Waiver
The Medical Assistance Waiver is for individuals 50 years of age and older who are eligible, both functionally and financially for nursing home placement. Through this program eligible individuals have an alternative to nursing home placement and can choose to remain in their own homes or assisted living facility. Senior Care
The Senior Care Program helps frail, health-impaired older persons select and locate the services that can help them continue to live in their homes. Personal care, light housekeeping, meal preparation, nursing care, shopping and transportation are among the services available. Services are made possible through the collaborative efforts of MAC or the local Commission on Aging, the Department of Social Services, and the local Health Department.
Each agency is committed to improving the coordination of services and filling gaps in home care for the elderly. After an evaluation is done in the home, a case manager ensures that the older person receives the right services at the right time.

- be 65 years of age or older need assistance with several daily activities (personal care, meal preparation, etc.)
- be medically at risk of institutionalization Senior Care has some funds available to help pay for gap-filling services for persons who meet established medical and financial criteria.
